Dear SEC Patients,

   Please download, print and review the following links and fill out the History, Physical and Insurance forms.  Bring the completed packet on the day of your appointment. 

   Also, please review the Consent Form but do NOT sign it. The Consent form is for your review and will be signed in the presence of a member of the nursing staff at the Sonterra Endoscopy Center .  

    We will call you on the day before your procedure to be sure that you understand what to expect and to answer any questions you may have.  Please be here 45 minutes to 1 hour prior to your procedure time, so that we can process your paperwork and prepare you for your procedure. Also please make certain you have a ride to and from the facility  as you will be here usually about 2 hours from start to finish, though this may vary.  

    Please feel free to call us at 403-3528 with any questions or if you wish, just ask your questions at the time that the nurse prepares you for your procedure. We appreciate you utilizing Sonterra Endoscopy Center .
